Millennium Weather Sign-up Form: Print, read the Terms of Agreement, complete form, sign, and return with payment to address shown at the bottom.

TERMS OF AGREEMENT:

You (and your organization) agree to indemnify and hold harmless Millennium Weather and Gary E. Gray for any action you or your organization may take in response to products or services provided. We are providing information only; we do not provide advice.
Signing this agreement entitles you to terminate services at any point in the contract if you become dissatisfied. A prorated refund will be issued.
You agree to keep confidential the password that provides access to our products and services via the Millennium Weather web site. Unauthorized redistribution of the password or products accessed via this password is prohibited.
You understand that failure to comply with the previous paragraph will result in a warning, accompanied by a password change. A second offense will result in the termination of the contract and forfeiture of any remaining funds on in the contract.
You agree that it is your responsibility to notify Millennium Weather of any e-mail address changes. Millennium Weather will neither reimburse funds nor extend contract length for failure of the user to receive e-mail products in the event that such a failure is due to non-notification of a change in the user's e-mail address.
Standard contract terms (not including research contracts) are for one calendar year in duration following clearance of your payment.
In the event of service outages, your account will be credited 150% of the lost time (e.g., a 10 day service outage during the contract term would result in a 15 day contract extension). You will be notified in advance of a planned outage. Unplanned outages will be followed up with an explanation.

ATTENTION: Emergency Service organizations, with prior approval, submit payment amount equal to one Tier lower than requested service level.
ATTENTION: For renewals, only fill in name and any information that has changed; sign, and submit with payment.
ATTENTION: For Tier upgrade, inquire for prorated payment difference then fill in name, sign, and submit with payment.
ATTENTION: Referal discounts will have received prior authorization. Apply discount and make note of it on form.

Submit this signed form with payment in full ($25 for Tier 1 service, $40 for Tier 2, $60 for Tier 3, $75 for Tier 4, or $100 for Tier 5). Payment may be made ONLY by check or money order in United States Currency and MUST BE MADE OUT DIRECTLY TO Gary E. Gray. Please allow two weeks from receipt of your form for check clearance (if necessary) and processing. Without a this signed document your service cannot be activated.
